HELP! Tough Pork Butt

I smoked a 7.4 lb boston butt over the weekend, tourned out terrible.
I used a rub called "All SOuth" I found on the net. Rubbed it and wrapped it in plastic over night. Used mostly Kingsford and some apple and hickory. Smoked it for 12 hours, it was only at 140 degrees internal and the outside bark was so tough you couldn't eat it. Do I need a water pan with this smoker? also; after 12 hours I would think it would be almost done?? ANy advice onthe tough bark and ot being doen would be appreciated.
